- A self-attested copy of proof of identity
Note:
If the passport includes the overseas address as mentioned on the declaration form,
then no additional proof of address is required.
- A self-attested copy of proof of overseas address (Any one of the following)
- Person of Indian Origin (PIO) card
- Overseas Citizen of India (OCI) card
- Latest electricity / telephone / water bill
- Other bank statement*
- Permanent driving licence
- Residence Permit / Government issued identity card
*Not older than 3 months, required for a period of 3 months with at least 2 self induced transaction and no bounced cheque reflecting in the statement.
- A self-attested copy of proof of Indian address (Any one of the following)
- Passport
- Election / Voter's ID
- Permanent driving licence
- Society outgoing bill
- Latest Electricity / Telephone / Water bill
- Property tax bill
- Domicile certificate with address issued by Municipal Corporation
- Gas bill (Pipeline connection)
- Registered rent/lease agreement (registered with State Government or similar regulatory authority)

- A recent passport size photograph
- Declaration form: Please submit the above mentioned documents along with the declaration form.
