List of documents:
- A self-attested copy of proof of identity (Any one of the following)
• Passport • Election / Voter's ID • Permanent driving licence
• Government employee ID card • Defence ID card
• Permanent Account Number (PAN) card • Aadhaar card • Photo ration card
• National Rural Employment Guarantee Act (NREGA) Job card
- A self-attested copy of proof of residential address (Any one of the following)
• Passport • Election / Voter's ID • Permanent driving licence
• Society outgoing bill (only from registered societies)
• Latest Electricity / Water / Telephone bill • Property tax bill
• Domicile certificate with address issued by Municipal Corporation
• Gas bill (pipeline connection only) • Registered rent/lease agreement (registered with State Government or similar regulatory authority)
- A recent passport size photograph
- Declaration form: Please submit the above mentioned documents along with the declaration.

Note:
• If the address on the document submitted as proof of identity, is the same as that updated in the
declaration form, that document will be accepted as valid proof of both - identity and address.
• If the address on the document submitted as proof of identity, differs from that updated in the
declaration
form, a separate document will need to be submitted as proof of address, as per the
options indicated above.
In case of add-on credit cards, please complete the declaration and provide us with the
above documents for each add-on credit Cardholder(s) in addition to the documents for
the primary credit Cardholder. |
Submission procedure:
Please submit these documents along with duly filled declaration form to avoid restrictions on the operation of your credit card(s). To submit the documents:
